It may be due to hyperplasia or carcinoma of adenoma(s). Undescended parathyroid glands are an infrequent cause of primary as well as secondary hyperparathyroidism (HPT); however, they are frequent in patients with recurrent HPT and with failed primary surgery. In cases of missed abnormal or ectopic parathyroid adenomas, imaging studies should precede surgery. The newer noninvasive imaging techniques have improved the preoperative (Preoperative) diagnosis of these undescended and ectopic glands. Here, we are presenting a case series of undescended parathyroid adenomas (UPAs) with the role of newer diagnostic imaging techniques and surgical procedures. 542Materials and methods: In our institution, we came across three cases of UPAs that were operated for PHPT. Their clinical, imaging, and surgical variables are discussed here. 543Results: Out of the three cases of undescended adenoma, one case was operated before for PHPT, and other two were virgin cases. Preoperative localization was done mainly with single-photon emission computed tomography (SPECT) and sestamibi scan. After the surgery for undescended adenoma, in all these cases, no complication was noted intraoperatively as well as postoperatively, and no recurrence was noted in ongoing follow-up. 544Conclusion: For diagnosing and treatment of UPA with PHPT, thorough knowledge of parathyroid anatomy and embryology, good imaging technique and focused surgical technique are necessary. Patients were subdivided into two cohorts based on preoperative diagnosis: (1) adrenal incidentaloma, defined as an adrenal tumor identified on imaging performed for a condition unrelated to an adrenal disorder; and (2) adrenal nonincidentaloma, defined as an adrenal tumor identified during evaluation for suspected adrenal disease. Statistical analysis included Chi-squared tests and multivariable logistic regression. 595Results: Of 224 patients who underwent adrenalectomy, 58.9% (n = 132) had adrenal incidentalomas and 41.1% (n = 92) had adrenal nonincidentalomas. DHGTC maintains the architectural and cytological features of well-differentiated follicular cell-derived carcinomas. However, it displays high-grade features, such as elevated mitotic activity and tumor necrosis. Herein, we present a case of DHGTC in a 76-year-old female patient, with compressive symptoms and discuss the histopathological features of this new entity. 657 658

--> 661

668 669 670

CASE REPORT
Elisabete do V Campos , 674 VÃtor Devezas , 675 João Capela , 676 João Pacheco , 677 Pedro Sá Couto , 678 Silvestre Carneiro 679
681 When a Thyroid Nodule is Actually an Unusual Intrathyroidal Thyroglossal Duct Cyst: Case Report 682

687 688 <!-- 689 <a href="javascript:void(0)" data-toggle="modal" class="citeModal modalAbstract" data-cite="Aims and background: Thyroglossal duct cysts (TDCs) typically present as midline neck masses located below the hyoid bone. The method comprises a Kocher skin incision, access to the thyroid gland, and mobilization of a thyroid lobe. To mobilize a thyroid lobe using a bipolar coagulator, a series of capsular microperforations of the thyroid fascial capsule are made sequentially. Each microperforation is 2â3 mm in diameter, spaced 5â10 mm apart. The length depends on the location of the microperforations. Microperforations are performed sequentially in the midlateral surface of the thyroid lobe, parallel to the trachea, for a length of 10 cm. Then, in the area of the upper pole and anterior suspensory ligament of the thyroid gland, for a length of up to 5 cm perpendicular to the trachea. Next, in the area of the lower pole of the thyroid gland near the thyrothymic ligament, for a length of up to 5 cm perpendicular to the trachea. After identifying and dissecting the thyrothymic ligament with ligation and transection of the thyroid ima artery (Ima) and vein, as well as their branches at the inferior pole of the thyroid gland, microperforations are performed in the posterior lateral surface of the thyroid lobe, extending up to 10 cm parallel to the trachea.
