We are so grateful to the students who take part and the schools and local authorities who have partnered with us.

524
The OxWell Student Survey started as a small project in Oxfordshire in 2019, working closely with the local authority, in particular public health and child and adolescent mental health services. Approximately 5,000 students in 56 primary and secondary schools participated. In 2020, the survey took place during lockdown and approximately 19,000 students in 237 schools participated, from 11 local authorities in the counties of Oxfordshire, Berkshire, Buckinghamshire, Gloucestershire and Wiltshire. This expanded participation allowed us to learn about a range of important mental health and wellbeing factors, including students’ experience of COVID-19, school closures and associated challenges.

525
In the 2021 survey, the area’s participating increased through the involvement of: Berkshire West; Buckinghamshire; East Berkshire (Frimley); Liverpool City; Oxfordshire; and Milton Keynes. More than 30,000 students participated from 180 schools.

526
The 2023 wave of OxWell survey had over 40,000 students participate from 185 schools and Further Education Colleges in 6 local authority areas. Over 200 questions were asked, providing us with invaluable information. We also designed a modified version of the survey in 2024 to ensure that non-mainstream schools could also take part, and we collected data from over 200 students.

527
The fifth wave of the survey took place in 2025 and over 35,000 students participated. We are working with Oxfordshire and Liverpool councils to gather information from as many young people as possible to help inform how to deliver the best possible health and education services.

We believe this helps us make sure that as many students as possible are able to participate. No students’ names, addresses or dates of birth are collected. This is intended to encourage open and accurate responses, including to some sensitive questions on self-harm and exposure to abuse while protecting participants’ privacy.

537

The target population for the OxWell Student Survey is children and adolescents attending school aged 9–18. To reach this group, OxWell recruits school students at state-maintained and independent schools and Further Education Colleges (FECs) in several local authorities across England. The survey aims to ensure a diverse and representative school population sample by three means: first, by recruiting students via schools in a range of socioeconomic deprivation areas; second, by using an opt-out model with parents to ensure maximum participation; and thirdly, by not trying to identify students to encourage accurate responses.

538
To assess diversity, the survey includes gender identity, ethnicity and and proxies for socio-economic status. To assess representativeness, analyses compare the survey sample to school-level information made available by the UK government, such as deprivation indices of the school location and the proportion of students accessing free school meals.

543

The OxWell Student Survey is designed to help us understand the relationship between social and behavioural risk factors, mental health outcomes and care-seeking in a school-aged population. There are around 80 questions in the core survey in 2025. They address a variety of contextual and lifestyle factors. There are two age-appropriate versions of the survey: one version is for Years 5 & 6 and one version is for Years 7 – 13 (including FECs).

544
We use some similar and some different measures of mental health and wellbeing in the different survey waves to allow comparison with baseline and potentially older surveys as well as refine and improve what we are asking as well as to be able to address current events and novel hypotheses. This ongoing development is informed by our research team, collaborators and stakeholders. We are especially grateful to the many young students who have helped us refine our measures and improve how we ask about key areas.

549

All primary schools, secondary schools and FECs in participating counties are invited via their local authority to sign up to participate in the OxWell Student Survey using an online form.

550
Schools that sign up to take part receive a study information pack from the OxWell research team. Schools then inform parents about the survey and provide them with instructions on how to opt-out their child, if they so wish.

551
During a designated school period, schools provide survey information (a 3-minute video) and login details to students who have not been opted-out. Students happy to take part then log in, read the instructions, provide online assent (or consent for those 16 and over) and complete the survey. To mitigate difficulties for younger pupils with the online assent process, students and school staff are informed about the assent procedure, the assent questions are presented one at a time, and students are instructed to ask their teacher if they do not understand a question. The entire session is expected to take most students about 15-20 mins, depending on the survey version (it is divided into two age-appropriate versions) and responses to ‘gateway’ questions that might probe participants with further questions.
